Social Model Daycare

Fort Tryon Center’s Social Model Daycare provides a structured environment where people with dementia or declining in cognitive ability can socialize and interact with one another in a secure, supervised setting, while all their day to day needs are being cared for by a well-trained and dependable staff. Door- to- door transportation is available and 3 nutritious meals and snacks are served daily.

  • Arts & Crafts
  • Discussion and Reminiscence Groups
  • Music

Group activities are designed to meet the individual needs and abilities of each member. Services are provided in an attractive, home-like setting, which is handicap accessible. Adult Day Care benefits everyone in the family and provides a much-needed respite from the physical demands and stress of providing around-the-clock care.

If eligible, Social Adult Day Care may be combined with long-term home health care.

Medical Model Daycare

Fort Tryon Center's Medical Model Daycare program is design for persons with medical problems, including dementia, who may require one or more of the following services:

  • Daily monitoring,
  • Medication administration
  • Nursing care,
  • Social work,
  • Occupational therapy,
  • Physical therapy,
  • Nutritional counseling
  • Recreational therapy
  • Socialization

The Medical Model Day Care program provides many benefits of a skilled nursing facility, while allowing participants to live at home.  In addition to medical assistance, participants receive help with all activities of daily living, including eating, bathing, grooming, toileting and ambulation. The program is open Monday-Saturday from 9 am – 3 pm.
